Parental Waiver Form
AKA: "Parent's Liability Release & Acknowledgment of Concussion Guidelines & Materials" Form

Required annually for players under 18. Only the player's parent or legal guardian may sign. Must be accompanied by parent's ID. Additional concussion resources are available here.

Referee Report Form

Referees must submit reports after each game. Check with your hiring league or assignor for detailed game reporting instructions. ALL reports containing descriptions of misconduct or player injuries MUST be forwarded to OASA within 48 hours!

Roster Form (Word) or Roster Form (PDF)

An updated roster form should be presented to the referee prior to each game.

Travel Papers Request Form

Affiliated tournaments outside of the state may require participating Oregon teams to submit Travel Papers. This form must be accompanied by a copy of your team roster. Requests received at least 30 days prior to the tournament start date will be processed for free!
