In accordance with Senate Bill 721 (“Jenna’s Law”), new concussion training and procedural guidelines are now in effect for Oregon coaches, managers, referees, players under the age of 18 and their parents or legal guardians. For each year of participation, and prior to a player under the age of 18 participating, at least one parent or legal guardian must acknowledge receipt and review of guidelines and materials related to concussions as described in the law. If the player is age 12 or older, the player must also acknowledge receipt and review of guidelines and materials.

1. E-MAIL Complete the fillable registration form, save it to your computer, then submit it as an e-mail attachment. Please be sure to include the parental liability release form and required identification. You may need to register for a free account (Adobe) in order to use the digital signature feature, otherwise, you can print the form, sign it by hand, then scan a copy and submit the resulting image via e-mail. 2. FAX Complete the registration form, print it, then submit it via fax, along with the parental liability release form and required identification. FOR NEW CARDS - YOU MUST ALSO EMAIL or MAIL A COLOR PHOTO. Our fax machine does not receive in color. Our fax number is 503-297-4513. |

Bring in a completed registration form or complete the form in our office, but don't forget you must also provide the signed parental liability release form and required identification. To access the registration, you must view our player waiver.
We try to process all registrations the same day they are received. Cards are typically mailed out the following business day. Please be mindful of the time it will take for your card to reach you by mail. If your next game is only days away, you should consider coming in to pick up your card in person. You are not registered until you have RECEIVED your card.
