Once you have served your suspension (typically determined by the league) you must pay a $20.00 administrative fee to OASA prior to the return of your player card. The league or competition in which you were playing may also fine you, this is separate from, and in addition to, any and all fines that may have been assessed.

You may fill out and return this form with your $20.00 fee and will follow your handling instructions below when your suspension is served.
